 Grace Healthcare provides a full range of services for the elderly or medically challenged individual.  Each skilled nursing facility is licensed under the particular state in which it is located and is managed by a licensed Administrator and Director of Nursing.    

An interdisciplinary care team (consisting of the attending physician, licensed nurses, social worker, activity director, and dietician/nutritionist) works with each resident and/or family member to develop an individualized medical care plan.  This plan of care is assessed frequently and changes made to ensure the resident reaches their optimum level of health and independence.
